| How to indicate your wish to donate your organs when you die It’s simple! You will find a sticker in the brochure “Please sign for life”, sign it and stick it on the upper back part of your health insurance card.
This brochure will be sent to you on the renewal of your health insurance card. It is also available in all Quebec hospitals, CLSC and pharmacies (Drug counter).

It is very important to notify your family members of your decision. That way, if death should occur, your wish will be respected .
It is not necessary to sign any other organ donation card; this sticker is sufficient to indicate your wish to donate your organs. By signing the sticker, you will accomplish one of the most generous gesture there is: allow a person to live by consenting to organ donation !
Have your decision recorded in the Registre des consentements au don d’organes et de tissus of the Chambre des notaires du Québec.
Making a will or a mandate in anticipation of incapacity is a perfect opportunity to consider organ and tissue donation.
If you consent to donate your organs and tissues after discussing the matter freely with your notary, he or she will record your consent in your
will or mandate. It will then also be recorded in a register designed for this purpose by the Chambre des notaires du Québec.
Through this confidential register, authorized personnel will be able to check at the appropriate time whether a person has agreed to donate organs and tissue.
